Dear Sir,

The report on "clinical stroke scores for distinguishing stroke subtypes" is very interesting. [1] Mwita et al. noted that "clinical stroke scores are not accurate enough for use in clinical or epidemiological settings" [1] and "computerized tomography (CT) is recommended for differentiating stroke subtypes." [1] It is admirable that Mwita et al. tried to search for new alternatives to help distinguish stroke subtypes in a rural setting. While the recommendation to use CT is rational, it might not be available in rural settings. The topic for discussion should be on how to improve the efficacy of stroke score. The additional use of other scoring systems, such as iScore, [2] which can be useful in distinguishing stroke subtypes should be considered.
